6th Grade Camp Information
Please complete, sign, and return the following required forms to your student’s teacher as soon as possible but no later than Friday, January 10, 2025.
* Pink Student Registration Form
* Blue Medication Authorization Form (If taking meds to camp)
* Payment (Checks should be made payable to: Twin Peaks Middle School.)
